First Aid Training Booking Form

Please Note: The minimum number for group bookings is six and the maximum is twelve.

By completing and returning this form you agree that you have read, understood and accept our terms and conditions of booking.

Terms and Conditions of Booking

The booking form must be completed for all courses reserved.
The balance of the course must be received at least one week before the course is due to take place, unless an agreement is made by prior arrangement between “the client” and MediRite UK First Aid Training.
If the course is cancelled, MediRite UK First Aid Training reserves the right to make the following charges:- 14 working days — 50% of the invoice value, 7 working days — full invoice value. Unless the course is re-scheduled to take place within four weeks of the original date.
Written notification is required for all cancellations / changes.
A maximum of twelve delegates will be allowed to attend both, the four day first aid at work, and the two day first aid at work Refresher course. A maximum of fourteen delegates will be allowed to attend all other courses unless stated by MediRite UK First Aid Training prior to the booking.
All first aid at work re-qualification course delegates are required to provide proof of current certification on or before commencement of the course, otherwise, a further full first aid at work course will be necessary, as stated by the Health and Safety Executive.
